The SE MN Area Agency on Aging (SEMAAA) is looking to partner with interested organizations to implement the Chronic Disease Self-Management Program (CDSMP), known as “Living Well with Chronic Conditions” in Minnesota. CDSMP is an Evidence Based program for adults with one or more ongoing health conditions such as diabetes, arthritis, high blood pressure, heart disease, chronic pain or anxiety. Workshop participants receive support, find practical ways to deal with pain and fatigue, discover better nutrition and exercise choices, and learn better ways to talk with their doctor and family about their health. The workshop is designed for groups of 8-15 adults, meeting once per week for six 2.5-hour sessions. The sessions are facilitated by two trained leaders. We will be holding a four-day CDSMP Leader Training in Oronoco, MN on March 23, 24, 30, 31, 2017. Potential leaders must attend all four days of training and facilitate a minimum of 1 workshop per year to maintain certification. Leaders must also agree to adhere to program fidelity standards. Mileage reimbursement is available for travel over 30 miles from leader training location for those traveling from within the 11 county SE MN region of Dodge, Fillmore, Freeborn, Goodhue, Houston, Mower, Olmsted, Rice, Steele, Wabasha & Winona Counties. The Leader Training is held by two Master Trainers, Lori Christiansen with the SE MN Area Agency on Aging and Linda Hoffman with the Owatonna Hospital Courage Kenny Rehabilitation Institute.

We are looking to increase the implementation of CDSMP in a sustainable manor, in conjunction with the goals of WellConnect, the SE MN Partnership for Community-Based Health Promotion. There is no registration fee for the Leader Training. Costs are covered by funds provided by the Healthy Living As You Age initiative. All start-up materials for workshop implementation will be provided.
