RETURN TO COMMUNITY CASE AIDE
PT position providing administrative and organizational support to the Community Living Specialists of the Return to Community Program. Work includes contacts to nursing homes, home care providers, seniors, caregivers and other partners. Assist to research and coordinate community supports and services that allow older adults to return to, or remain in, a community setting. Will be working with a Voice IP system and multiple data collection tools. Assist with data entry, triaging of inquiries or referrals and outreach events. Preferred candidate will have 2-4 year college degree and experience in customer service, county, nursing home or health care settings with knowledge of area support services desirable. Community Living Specialist
Full-time work assisting older adults to access community supports and services that allow them to return to, or remain in, a community setting. Will be working with nursing home discharge planning teams, home care providers, seniors and their caregivers. Work includes on site in-depth assessment of emotional, cognitive, social, financial & functional skills. Will assist in service arrangements, goal setting, care planning and face-to-face follow-ups. Must have a BS/BA degree in either Social Work, Nursing or other related human services field. Previous work in care coordination, case management & at least 3 years of work experience in county, nursing home, or health care settings.
